This position is responsible for a broad range of administrative, teaching, research and clinical activities essential to the development and operation of the Department's Geriatrics program. In addition, this position will work closely with the Residency Program Director for teaching activities as they relate to the Springfield Family Medicine Residency Program.

TEACHING (20%)
A. Supervise residents and medical students in patient care activities at the Center for Family Medicine. B. Conduct teaching rounds on hospitalized patients. C. Provide teaching and modeling to fellows, residents and medical students on the principles, benefits and core concepts of interdisciplinary team care for frail geriatric patients with complex needs. D. Work with geriatric interdisciplinary team members to define key outcomes expected from interdisciplinary care (i.e. Maintenance of function, decreased falls and delirium, fewer foleys and inappropriate medication, decreased LOS, fewer admissions in 30 days, increased patient and family satisfaction, infection control, etc.). 2 E. Actively precept as teaching physician in the resident outpatient clinics to include patient care aspects and procedural skills, time management skills, doctor-patient relationship, professionalism, ethical and legal considerations, billing and coding. F. Set foundation and structure for possible Geriatric Fellowship Program.
SERVICE (Clinical) (45%)
A. Serve as Consultant for Geriatrics and Palliative Medicine referral clinic and the Geriatric Interdisciplinary Assessment clinic. B. Participate in special clinics such as the Frailty Clinic, Falls Clinic, Bone Density Clinic, etc. C. Participate in teaching service rounds a minimum of five weeks per year and family medicine call. D. Conduct nursing home rounds with residents and participate in assisted living rounds with the Geriatric team as needed. E. Provide emergency services to patients as needed including those who are identified as having no primary care physician.
ADMINISTRATION (20%) A. Serve as Director of Geriatric and Palliative Medicine. B. Administer the Geriatric and Palliative Medicine Programs and set priorities for providers and staff assigned to these programs. C. Work with Geriatric Team to provide oversight over all Geriatric curriculum development, implementation and evaluation. D. Recommend policies and procedures and quality control procedures for the Geriatric Program. E. Work collaboratively with affiliated hospitals and the Department of Internal Medicine to develop and expand Geriatric programs and services. F. Serve as Director of Sports and Musculoskeletal Medicine for the Springfield site. G. Work with Geriatric Team to provide oversight over all Geriatric and Palliative Care curriculum development, implementation and evaluation. H. Serve as Medical Director for affiliated nursing homes. I. Identify, implement and model "best practices" in the field of Geriatric medicine. J. Supervise the Geriatric Program's advanced practice professionals and staff.
RESEARCH (10%) A. Contribute to the identification and implementation of appropriate research projects and publications in the area of Geriatrics and Palliative Medicine and Clinical Epidemiology B. Per RRC requirements, must complete a minimum of 2 scholarly activities every 5 years. 3
SERVICE (OTHER PROFESSIONAL ACTIVITIES) (5%) A. Attendance/presentation at national professional meetings. B. Serve on the Center-wide Committees, as assigned. C. Serve on school and department committees, as assigned. D. Participate in community outreach projects. E. Attend faculty meetings and resident conferences F. Participate in Center Peer Review and Quality Improvement activities.

REQUIRED QUALIFICATIONS:

1. Professional Medical Degree or foreign equivalent. 2. Board certification in Family Medicine at time of hire 3. Licensure to practice medicine in the State of Illinois 4. Current certification in Geriatrics from ABFM. 5. Must be sensitive to the needs of underrepresented minority populations.
