Position Summary

The Social Services Family Support Worker (FSW) is responsible for all clinic psycho-social referrals, screening and assessment, and short-term interventions and education. A key component of the position is to coordinate services with the medical, dental, and behavioral health providers, as well as collaborate with outside service providers. This position is hybrid, which requires in-person and remote services. Bilingual in Spanish & English or Amharic & English preferred.

Reportability

This position will report directly to the Family Support Worker Supervisor.

To perform this job successfully, an individual must be able to perform each essential duty satisfactorily.

Case Management and Care Coordination:

  • Provide direct services to participants encompassing a range of psycho-educational and social supportive services, while conducting ongoing assessment of participants, and developing a care plan as needed.
  • Include relevant family members in the treatment planning process; ensure all referrals are documented and outcomes are tracked.
  • In conjunction with medical, dental, and programs staff provide short, brief problem specific case management services according to participant need/treatment plan.
  • Serve as the case manager liaison among the participants and other members of the organization.
  • Support medical team with participants identified with medical risk to include ensuring continuity of care.
  • Complete a psychosocial assessment on all participants referred, including domestic violence, social determinants of health, depression, and substance abuse screens.
  • Provide options counseling and prenatal support services when needed.
  • Gather information from referral source, participant chart and other pertinent sources in completing the assessment (including outside agencies).
  • Identify possible barriers to participant care and support in identifying community resources to reduce such barriers.
  • Discuss updates on participant's progress and/or barriers with supervisor.

Documentation:

  • Enter all screenings, assessments, encounters, and referral information in appropriate health record and/or data system(s).
  • Ensure service note entries are documented in electronic health records within 48hrs from date of service.
  • Ensure that all applicable forms are complete, proofread for accuracy, reviewed and signed-off by supervisor and uploaded into electronic health record.
  • Complete all reports as required based upon the timeline provided.

General:

  • Provide and model outstanding customer service to all participants.
  • Follow HIPAA confidentiality guidelines.
  • Work together with their supervisor to plan and achieve desired performance goals.
  • Perform other duties as determined necessary by their supervisor.

Knowledge, Skills & Abilities -

  • Bachelor's degree in Social Work, Public Health, Gender Studies, Human Services, or related field preferred.
  • At least 3 years of relevant work experience
  • Must possess knowledge of community resources, and a working knowledge of community support and referrals.
  • A thorough understanding of patient/client confidentiality issues and HIPAA compliance/regulations.

Language Requirements - Must be professionally fluent in Spanish and English or Amharic and English.

Physical Demands - Regularly required to sit; frequently required to reach with hands and arms, walk, stoop, kneel, crouch, talk or hear; must be able to lift objects up to twenty-five (25) pounds.

Work Environment - Mostly in a typical office and/or home setting with quite to moderate noise level. May be exposed to extreme cold, heat, and humidity due to outside weather conditions. Hybrid schedule - remotely & onsite, as needed.
