Job Description

Reporting to the Cardiopulmonary Rehabilitation Manager, is responsible for the care of patients in the Cardiac and/or Pulmonary Rehabilitation continuum by case managing assigned patients from initial consultation through discharge from the program. Continually assesses patients and implements treatment as necessary. Involves family/guardian in the treatment plan. Assumes responsibility for the development of exercise prescriptions for patients.

II. REPRESENTATIVE FUNCTIONS: (listed in order of time spent):

Supervises assigned exercise sessions in outpatient cardiac and/or pulmonary rehabilitation, including: developing and implementing exercise prescriptions in order to increase the functional capacity of the cardiac patient safely and effectively. Monitors patients for inappropriate responses to exercise.

Leads warm-up and cool-down exercises. Evaluates patient progress and creates or modifies individual goals and objectives.

Conducts patient education in inpatient and outpatient settings considering the readiness of the patient to learn. Provides inpatient education to patient and family regarding daily activity and exercise guidelines, risk factors, pulse taking, and activity limitations. Provides outpatient education to group classes in accordance with departmental education program. Provides outpatient education on an individual basis according to patient needs.

Maintains patient records, progress reports, and documentation per policy and procedure manual and Medicare guidelines.

Maintains emergency preparedness through departmental in-services and certifications.

Participates in department and hospital projects, committees, and special events as desired or assigned. May be called upon to participate in cardiovascular screening programs.

Maintains physician and peer relationships that demonstrate support, mutual respect, collaboration, and goal-directed interactions.

Adheres to all departmental and Northwest Community Hospital standards, policies, and procedures.

III. SPECIAL SKILLS AND ABILITIES REQUIRED:

The interpersonal skills necessary to interact effectively with patients, families, and all levels of hospital personnel.

The communication skills necessary to present information in a clear, concise, and persuasive manner.

The enthusiastic and energetic personality and positive image of good health necessary to motivate others to begin and maintain health behavior changes.

The analytical skills necessary to assess patients and formulate care plans.

The ability to interpret heart rhythm strips.

The ability to develop exercise prescriptions for cardiac and pulmonary rehab patients.

IV. KNOWLEDGE, PRACTICAL EXPERIENCE AND LICENSURE/REGISTRATION REQUIRED:

Bachelor's Degree in Exercise Physiology with a specialization in cardiopulmonary health and rehabilitation required. Master's degree preferred.

Minimum one-year experience in a multi-phase cardiopulmonary rehabilitation program, including developing exercise prescription.

Current BCLS certification for healthcare providers required.

ACSM Certified Clinical Exercise Physiologist and/or AACVPR Certified Cardiac Rehabilitation Professional certification preferred.
