LSSI is hiring afull-timeCounselor/Case Manager.Provide moderate-to-intense community-based case management services for individuals with serious mental illness to promote community functioning and decrease psychiatric hospitalizations and crisis episodes.

EssentialFunctions:
  • Provide and coordinate a continuum of treatment services.
  • Carry a caseload of clients providing supportive counseling and concrete services as assigned by Supervisor and operate as part of a treatment team.
  • Ensure all documentation is completed in a timely manner and in accordance with Rule 140 requirements, DHS, and other funding/regulating bodies.
  • Transportation of clients in personal vehicle and agency vehicle is required. Services will be provided in multiple locations.
  • Other duties assigned.
Position Qualifications:
  • Bachelor's degree required, Master's preferred.
  • 1-3 years of clinical experience preferred.
  • Experience in Crisis Intervention and De-escalation.
  • Excellent written and verbal skills.
  • Valid Illinois Driver's License & Insurance.
