Description
TheSocial Worker LSW or LCSW reflects the mission, vision, and values of NM, adheres to the organization's Code of Ethics and Corporate Compliance Program, and complies with all relevant policies, procedures, guidelines, and all other regulatory and accreditation standards.
Responsibilities:
Responsible for providing discharge planning and psychosocial interventions to patients and their families.
Responsible for providing social work consults to the emergency room and Primary Care Center and other outpatient areas / facilities. Responsible for providing on-call coverage.
Responsible for providing age appropriate and culturally appropriate patient care.
Responsible for participating in specific department activities. Responsible for participating in professional activities to enhance social work skills. Responsible for professional conduct.
This position is full-time, 80 hours per pay period, day shift with alternating weekends and holidays.
