Job Description:

SUMMARY OF POSITION

Provides day-to-day supervision and living skills instruction for the residents and carries out duties as assigned by the House Manager or Residential Coordinator. This includes providing training and support for residents to help them attain maximum independence and maintenance of appropriate documentation. A Residential Supervisor is required to use independent judgment (or works under the direction of supervisor) when performing job tasks/providing services. As part of Centerstone's team-oriented philosophy, a Residential Supervisor is expected to actively participate in variety of cross-functional teams and performance improvement initiatives.

ESSENTIAL DUTIES

  • Assists service recipient in developing an individualized treatment plan, must be person centered, family centered, and service centered. Monitors service recipient progress in treatment and participates in treatment plan reviews as assigned. Updates treatment plans as appropriate. Submits treatment plans at all stages for medical review, if applicable. (clinical/direct service).
  • Assists service recipients with making linkage with internal and external resources as outlined in treatment plan and in coordination with Integrated Healthcare Services.Maintains appropriate contact with referral sources including providing information on service recipient progress, behavior, and needs (with appropriate releases), and maintaining ongoing consultation when appropriate.
  • Provides transportation for service recipients as needed.
  • Provides personal care/physical assistance as needed.
  • Attends staff meetings, inter-disciplinary staffings, and other internal meetings as assigned. Participates on committees as assigned, including quality assurance functions. Maintains appropriate communication with other employees and work as an effective member of treatment team.

RESPONSIBILITY

Supervisory Controls: The Residential Coordinator and/or House Manager assigns work priorities, provides ongoing supervision, and interprets and enforces policies and procedures.

Guidelines: Board, Administrative and program policies, licensure, accreditation, statutory and regulatory standards restrict the position's discretion in completing job tasks. Staff has the prerogative to propose changes in practice, procedures and policies that affect their team.

DIFFICULTY

Complexity: Position is characterized by a series of varied job tasks, and flexibility is required for dealing with situations as they arise.

Scope & Effect: This job affects the physical, emotional, and psychological well-being of service recipients.

PERSONAL RELATIONSHIPS

A. Personal Contacts: Position has daily contact with residents, House Manager and Residential Coordinator regular contact with medical providers, Assistant Director, Program Director and community members and may have occasional contact with donors, advocates and guardians.

Purpose of Contacts: Purpose of contact is primarily for facilitating resident services. Outside contacts are for linking residents with other community services and promoting community integration.The purpose of contact with others is primarily to gather and exchange information.

ENVIRONMENTAL DEMANDS

Physical Requirements: While the work is somewhat sedentary, it may require considerable standing, climbing stairs, lifting or carrying of moderately heavy items, and general house and yard maintenance. In homes with mobility-impaired clients, personal care including lifting up to 75 pounds, transferring, re-positioning and otherwise strenuous activities are required. Skill in non-violent crisis intervention (CPI) is required.

Work Environment: Work is primarily performed in a residential environment, but will include time in the community at location not limited to physicians' offices, restaurants and stores. There is risk due to location of services and/or potential violence of service recipients in crisis. Position may require contact with bodily fluids; this requires the use of protective clothing.

MINIMUM CREDENTIALS/REQUIREMENTS

Must pass spelling test, grammar test, and required background clearance checks (post-offer physical and other background checks) prior to hire. Orientation to population, and high school diploma or GED required. TB testing also required.

For those working in MI residential, must meet requirements for RSA designation (must be at least 21 years of age).

For those working in I/DD residential, must meet requirements for DSP (Direct Support Person) designation and Rule 116 requirements for medication administration within 90 day probationary period.

Must obtain CPR/First Aid certification and pass required training within 90 days probationary period, and maintain thereafter. Must possess a valid Driver's License and a good driving record. Residential Supervisors utilizing electronic health records and/or other Automated/electronic documentation systems are required to have computer and keyboarding experience and skills or the ability to develop such skills within required timeframes in order to meet the level of proficiency set by the agency. Residential Supervisors utilizing electronic/automated documentation systems will be required to undergo a computer and keyboarding assessment test to determine proficiency.

This role has a Wage Progression within first 90 days as the following trainings provided by Centerstone are completed:

Entry - $17.00

DSP Certified (30 days) - $17.60

Med Authorized (90 days) - $18.25

Level: 7 (both MI & DD as of 1/1/14)

DOL Status: Non-exempt

HBV Level: I
