Job Description:

SUMMARY OF POSITION

Promotes individuals and community partners in developing healthy lifestyles and increasing positive functioning. This includes providing direct support for placement stabilization to caregiver (foster parent) of child/youth for whom DCFS has legal responsibility. Communicates and coordinates resources to best meet foster family needs. Including facilitation of needs assessment, collaboration during clinical intervention placement preservation and team meetings. Provides education in Trauma Awareness ARC to foster parents, and tracking of client, foster parent and caseworker surveys.

ESSENTIAL DUTIES & RESPONSIBILITIES

  • Assigned participation in CIPP (Clinical Intervention Placement Preservation) meetings, providing collaboration for State of Illinois with Placement Preservation services.
  • Required and Obtained CANS certification (Child and Adolescent Needs and Strengths), conducted on incoming clients.
  • In accordance with DCFS standards, ARC (Attachment, Regulation and Competency) Trauma Awareness Training and Certification including continuous monthly reflection meetings.
  • Collaboration and presentation of IPS program with all requested partnering agencies including DCFS (Dept. of Children and Family Services), and local POS Agencies (LSSI and CARITAS).
  • Provide Referral and Linkage for Psychological Evaluation and Psychiatric Services assistance to caseworkers.
  • Attend Psychiatric Staffing's as requested by the State.
  • Mandated trainings by DCFS in regards to computer based reporting system, DCFS Database and inputting direct services into both Centerstone Avatar and DCFS Database.
  • Providing Interim Care Plan, including Emergency Services Information,for those put on Waiting List for therapeutic services.
  • Required Participation in Child and Family Team Meetings with foster parents and community service providers.
  • Provide Foster Parent Training Quarterly (4 annually).
  • Order and maintain resources for Foster Parents/ Support Groups/ Parenting Workshops.
  • Conduct Home Visits, Foster Parent Needs Assessments, Supportive Collaboration, Post-Surveys for Client, Foster Parent and Caseworker.
  • Plan and schedule appropriate aged grouped foster activities to promote social-emotional development.
  • Attend regular Case Staffing's to plan, monitor, revise and address any foster family service needs.

KNOWLEDGE, SKILLS & ABILITIES

  • Effectively communicate via written, verbal, in person and virtual methods.
  • Ability to manage communication between individuals with varying levels of training and knowledge.
  • Knowledge of foster care system a plus.

QUALIFICATIONS

Education Level

Bachelor's degree in appropriate field of Human Services, Education, related field orAssociates Degree plus required three years of experience and knowledge base of childhood development, child care, and parenting issues.

Years of Experience

  • Required three years of knowledge base of childhood development, child care, and parenting issues. Also, three years of home visitation and community outreach required. Knowledge of foster care system a plus.
  • Three years of home visitation and community outreach required.
  • Providers utilizing electronic health records and / or other automated/electronic documentation systems are required to have computer and keyboarding experience and skills of the ability to develop such skills with required timeframes in order to meet the level of proficiency set by the agency.

Certification/Licensure

  • ARC Trauma Treatment for Children and Adolescents Certificate Course and CANS Completion within 6 months of hire.
  • Must have valid driver's license.

Entry Pay Range:

$15.11 - $21.17
