Position Summary Information

Job Description Summary
Full-time, grant funded Clinical Care Coordinator to assist with care coordination and outreach activities for the Lewis student population
Responsibilities
Responsible to model the University Mission through dedicated job performance, service excellence to constituencies, respectful collaboration and active support of the University's Mission and Catholic and Lasallian higher education.

Care Coordination and Clinical Mental Health Services
-Support care coordination of students, connection to resources and relationship with service partners and referral sources
-Offer urgent care, crisis intervention and follow-up to promote safety and well-being of students
-Provide short-term individual and group counseling and education
-Maintain appropriate clinical documentation for client, consultation and group services

Curriculum Development and Training
-Responsible for identifying and implementing programming/outreach activities specific to the grant utilizing peer educators, social marketing, new student programs, and student organizations
-Organize suicide prevention and substance use prevention activities with a lens towards health equity in collaboration with the Coordinated Community Response Team/Campus Violence Prevention Committee and Student Wellness Center.
-Establish and maintain ongoing partnership with key local agencies for future collaboration towards program development.

Faculty and Staff Engagement
-In coordination with the grant leadership team, identify key faculty and staff to participate in a Train the Trainer training
-Conduct QPR training with campus partners
-Develop a marketing campaign to raise awareness of referral resources and supports

Grant Administration
-Prepare status and progress reports of ongoing outreach activities completed towards Suicide Prevention efforts.
-Maintain and report activities as outlined by SAMHSA Grant requirements and in coordination with grant leadership team.


Minimum Qualifications
-Master's degree in social work, counseling or related field
-Licensed in the State of Illinois
-Experience working with at-risk youth in community or education settings.
Knowledge in prevention, including theory and practice

Skills and knowledge required
-Organizational skills
-The ability to work independently
-Communication, both in public and in writing
-Possess sound experience with computer software programs
-Knowledgeable of current mental health and suicide trends
-Basic knowledge of data collection and analysis
Preferred Qualifications

8:30am - 5pm, some evening and weekends required.
