--FOR COMMUNITY SERVICES--

Essential Job Functions:

  • Be knowledgeable of Preferred Family Healthcare (PFH) Policies and Procedures, and Community Services procedures..

  • Be knowledgeable of PFH Community Services procedures.

  • Maintain confidentiality of Protected Health Information as directed in HIPAA training.

  • Coordinate and participate in the development of the Individual Support Plan (ISP); ensure outcomes are measurable and clear to staff.

  • Ensure staff are trained in the individual's written plan and in their responsibility for implementing the plan.

  • Ensure accurate and timely documentation is completed for authorized services and supports.

  • Supervise and evaluate support staff.

  • Conduct, at a minimum, quarterly Record and Therapy Reviews to ensure records are current, complete and contain substantial documentation that services were provided as authorized.

  • Make weekly visits to the ISL to monitor the status of the individual, complete PC weekly checklist, make sure of the health of the individual, and make sure premises are safe and clean.

  • Maintain client profile in database, enter staffing pattern and prepare annual ISL budgets.

  • Track monthly ISL budget exceptions and prepare end of year variance reports.

  • Ensure all staff have current training, including any training specific to the individual and updated as changes occur.

  • Work in conjunction with Primary Support Staff and Residential Nurse to ensure medical needs are taken care of, all medicines and treatments are given as ordered by physician, and encourage wellness such as good diet, exercise, and healthy habits.

  • Complete daily documentation audits in Therap to include health, meds and daily notes, goals and GER (General Event Reports).

  • Coordinates the provision of medical care to those served and encourages wellness such as good diet, exercise, and healthy habits.

  • In residential settings, ensure money counts and controlled drug counts are being done at the beginning and end of each shift.

  • Complete Monthly Reviews on an individual's progress or lack of progress on goals and objectives in the ISP, and overall status of the individual.

  • Facilitate opportunities for natural support and community participation.

  • Other duties as assigned

Knowledge, Skills, and Abilities:

  • Must have a reliable means of communication;

  • Must possess skills to read and understand written materials;

  • Must possess skills to write clearly and legibly;

  • May be required to assist with physical transfers or lifting, depending on the needed supports of the individual;

Experience and Education Qualifications:

  • A bachelor's degree from an accredited college or university.

  • Relevant experience in the field of Developmental Disabilities can be substituted year for year for a degree.

Supervisory Requirements:

  • The Program Coordinator will directly supervise Community Service Managers, Primary Support Staff and Direct Support Staff.

  • Communicate the strategic direction of the organization and encourage participation by all associates.

  • Provide leadership and guidance to all aspects of services provided.

  • Take an active role in monitoring the identification, development, and execution of strategic objectives.

  • Involve, as appropriate, all associates in order to achieve goals.

  • Effectively communicate to associates any changes and news worthy events within the department or company to all associates.

  • Handle difficult personnel situations directly, using appropriate discretion and Human Resource advice to show respect for the individual

  • Champion change and effectively manage the implementation of new ideas.

  • Reinforce team approach throughout functions; support and solicit input from team members at all levels with the company.

Employment Requirements:

  • Successful completion of background check including criminal record, driving record and abuse/neglect.

  • Completion of New Hire Orientation at the beginning of employment.

  • All training requirements including Relias Learning at the beginning of employment and annually thereafter.

  • Must have appropriate, reliable transportation, proof of insurance, and a valid driver's license and registration.

  • Must have working air conditioner and heater in any vehicle used to transport clients.

  • Will be on-call according to the regional office schedule.

Physical Requirements:

  • The physical demands vary depending on the support required by the client. While performing the duties of this job, the associate at times will be required to stoop, bend, use fine motor skills in extremities for cooking, cleaning, medication administration and operation of a vehicle, etc. The associate may occasionally lift and/or move up to 35 pounds.

  • Must be physically able to perform CPR (as instructed in the CPR manual) and First Aid.

  • May be required to assist with physical transfers or lifting, depending on the needed support of the clients.
