Job Description

Heritage has full-time positions that work with our treatment teams on providing both onsite and community-based nursing services, including but not limited to residential care, case management, care coordination, and linkage services for individuals with mental health, substance use, and co-occurring disorders. The programs work toward supporting individuals served to maintain recovery in the community. These positions will provide services in various settings, including individual homes, community settings, community agencies, healthcare providers, and Heritage.

Sign-on Bonus Available for LPN/RN positions!

Community Based Positions available include;

  • Assertive Community Treatment- LPN - $5,000 sign on bonus available
  • Assertive Community Treatment- RN - $7,000 sign on bonus available
  • Community Outreach-LPN - $2,500 sign on bonus available

Onsite Positions available

  • Integrated Healthcare Clinic - LPN - $2,500 sign on bonus available
  • Medicated Assisted Recovery - LPN - $2,500 sign on bonus available
  • Provider Support LPN- Dewitt Office -
    • 20 hours (part time)
  • Recovery Residential- LPN - $5,000 sign on bonus available
    • 12 hour shifts; 7:00 a.m. to 7:00 p.m. and 7:00 p.m. to 7:00 a.m.
    • Every other weekend availability required

Responsibilities Include:

  • Engage the individual served in their care in community settings (home, nursing facilities, public places, agencies, group homes, healthcare organizations, etc.).
  • Identifies key health indicators to monitor an individual's wellness and healthcare needs.
  • Use of clinical tools, data and other information to complete assessments of patient needs
  • Clearly explain the reason for prescribed treatments and treatment plans to patients
  • Communicate with patients, physicians and insurance companies to secure successful outcomes
  • Engage individuals served in wellness assessments at established timeframes.
  • Collects, assesses, and monitors vital checks, including blood pressure, blood sugar, and weight) at established timeframes
  • Responsible for ensuring individuals served referral, linkage, and attendance to annual medical evaluation visits, specialty health care visits, and any other health-related activity
  • Consultation and collaboration with all healthcare providers to ensure the sharing of information among all providers
  • Request and confirm receipt of all evaluations, labs, testing, and healthcare to allow for follow-up treatment
  • Communicate effectively with other providers, staff, and individuals served.
  • Participate in treatment team meetings with shared providers to collaborate on how to assist the person served (s) in their recovery
  • Individual and group interventions to provide training, education, and support to those with behavioral health, health conditions, and/or to help prevent health conditions
  • Individual and group activities, including physical exercise such as walking, yoga, smoking cessation, healthy living, diabetes self-management, and other wellness activities
  • Completion of all documentation and tracking promptly
  • Provide other duties as assigned, required, or specifically negotiated with the supervisor.

Knowledge, Skills, and Abilities:

  • Ability to work with individuals with a mental health and/or substance use diagnosis;
  • Knowledge of community resources and/or ability to research/find;
  • Knowledge of case management techniques;
  • Knowledge of healthcare resources, including psychiatric, substance use, medical, and/or other services;
  • Ability to meet people where they are at in their recovery, non-judgmental, and ability to build rapport quickly;
  • Ability to work as a team player;
  • Ability to tolerate ambiguity, uncertainty, and change;
  • Ability to navigate electronic health records;
  • Ability to work collaboratively with the direct leaders and other clinical staff providing services to the person served
  • Ability to respond to supervisors, person served, and other clinical staff providing services to the person served.

Education and Experience:

The ideal candidates will have experience working with individuals in behavioral health settings.

  • Current CPR certification per American Heart Association guidelines
  • Current undisciplined licenses as a Licensed Practical Nurse (LPN) in the state of Illinois
  • Current undisciplined licenses as a Registered Nurse (RN) in the state of Illinois
  • Current Certified Medical Assistant certificate in the state of Illinois
