This position serves in an expanded management role and is a part of the SIU Center for Family Medicine Federally Qualified Health Center core administrative team. The SIU FQHC spans several clinic sites including Springfield, Quincy, Jacksonville, Lincoln, Taylorville, Decatur, Carbondale, and Pittsfield. This position serves the role of coordinating the quality and risk management plan for all sites under the FQHC and is responsible for a model of care that promotes the delivery of safe; quality, efficient, and cost-effective healthcare services to patients. This includes monitoring progress and promoting early intervention, and managing utilization and practice metrics to maximize clinical quality.

Examples of Duties

Administration of the Quality Improvement/Risk Management 75%
1. Work with health care teams in all sites to continuously evaluate process, identify problems, and propose improvement strategies to enhance delivery of care model.
2. Staff the FQHC Board Quality Improvement Committee.
3. In collaboration with the Deputy Director and Chief Medical Officers, develop clinical guidelines and policies.
4. Responsible for keeping abreast of NCQA Patient Centered Medical Home guidelines and coordinating designation for both sites.
5. Assist with Meaningful Use measures and with obtaining data for reporting to HRSA using the Uniform Data System.
6. Meet regularly with FQHC administration to review and problem-solve clinical/operations process issues.
7. Supervise quality nursing staff that work to improve and enhance care processes to better meet patient needs.
8. Responsible for monitoring patient satisfaction surveys, identifying problems and trends.
9. Oversee PDSA cycle studies and provide feedback and data on outcomes and recommendations.
10. Oversee HRSA required Peer Review Process in conjunction with the Medical Director, to ensure all sites are meeting their obligation.
11. Facilitate GEMBA board review, and engage teams in pre-visit planning and clinic huddles.
12. Develop quality dash boards and share outcome and performance data with administration and providers.
13. Serves on Risk Management Committee and department-wide PSES Committee.
14. Enhance systems for the reporting of adverse events, near misses, and potentially unsafe conditions. Reporting responsibilities may include internal reporting as well as external reporting to regulatory, governmental, or voluntary agencies
15. Collect and analyze data to monitor the performance of processes and implement patient safety initiatives and strategies to minimize risk such as timely documentation, test tracking and systems related to higher risk medications such as Coumadin.
16. Implement programs that fulfill governmental, regulatory and accreditation requirements and ensure compliance of data reporting requirements.
17. Proactively advise the organization on strategies to reduce unsafe situations and improve the overall environmental safety of patients, visitors, staff, and volunteers.
18. Facilitate and ensure provider and staff participation in educational programs on patient safety and risk management.
19. Promote a culture of safety in the organization that embodies an atmosphere of mutual trust in which all providers and staff members feel empowered to report safety concerns and provide potential solutions without fear of retribution.
20. Assist in developing a patient and family communication and education plan that improves interactions with involved parties related to adverse or untoward events.
21. Assist in the completion of the Federal Claims Tort Act deeming applications.
22. Work with teams to strategize more efficient and effective handoff processes for continuity of patient care.
General Administration/Outreach 25%
1. Hire, evaluate and directly supervise quality staff in unit.
2. Indirectly supervise/direct staff working on quality projects in all health center sites.
3. Service on SOM/Medicine committees and FQHC related stat or national committees as applies.
4. Performs duties as required or assigned by emergency or other operational reason for which the employee is qualified to perform.

Qualifications

1.) Bachelor's degree, as required by the position to be filled in the area of specialization inherent in the position* AND

2.) 3 years (36 months) of Quality Improvement (QI) work experience in a clinical or healthcare related field/setting performing duties comparable to the Quality Specialist level of this series

* All candidates must possess a Bachelor's Degree in order to meet the Minimum Acceptable Qualifications.

Condition of Employment: Pursuant to the State Universities Civil Service System, an out-of-state resident who is hired into this position must establish Illinois residency within 180 calendar days of their start date.
