The Intake Specialist (Certified Community Behavioral Health Clinic) CCBHC is responsible for the initial clinical record and service level recommendations of all new referrals seeking services.

ESSENTIAL DUTIES & RESPONSIBILITIES
  • Responsible for the initial clinical record and service level recommendations of all referrals seeking entrance into Thresholds CCBHC for services.
  • Completes the preliminary screening and risk assessment, initial evaluation, comprehensive person-centered and family-centered diagnostic and treatment planning evaluation. Works in consultation with the program leadership to assess for safety and completes safety assessments when indicated.
  • Provides intake, substance use screening and assessment, and mental health assessment services on-site at the clinic and in the community which includes at referral residences, psychiatric hospitals, nursing homes, Specialized Mental Health Rehabilitation Facilities (SMHRF), public spaces and other locations as needed.
  • Responsible for day-to-day management of schedule of assessments as well as additional clinical services including coordination between treatment teams and, when necessary, other components of the agency. This may include coordination of treatment and treatment planning.
  • Intervenes and works cooperatively with collateral sources including but not limited to designated collaborating agencies, inpatient hospital staff, agencies, bureaucracies, facilities, and families on the members' behalf. Must keep logs and other records of all contacts as required by agency or program practices.
  • As needed, consults with leadership staff and others as designated on level of service, diagnostic impression, and prognosis and safety mitigation; elevates safety concerns to Thresholds' High Risk Committee.
  • Provide direct service to members. This includes, but is not limited to, group counseling/therapy, case management, assertive community treatment, job support, crisis intervention, and psycho-social rehabilitation services. This may be done in the community or on-site at a Thresholds location.
  • Other duties as assigned.
MARGINAL DUTIES & RESPONSIBILITIES
  • May be required to work a flexible schedule and be available evenings and weekends.
  • May be required to provide transportation for members of the program using a personal vehicle.
  • Performs and participates in crisis intervention when necessary.
EDUCATION
  • Master's degree in social work, counseling or closely related field is required.
EXPERIENCE
  • Minimum of two (2) years of experience with conducting behavioral health assessments.
  • Ability to work cooperatively with other team members, their support systems, collaborating agencies, other co-workers, and management.
  • Computer skills are required, including knowledge of database functions and maintenance.
SKILLS/CERTIFICATIONS
  • LPC or LSW preferred.
  • LCSW or LCPC is preferred.
  • Daily access to a car required.
  • Valid driver's license required.
  • Must obtain and maintain liability car insurance of:
    • $100,000 per person bodily injury
    • $300,000 per accident
    • $100,000 property damage
