Job Description

OFFERING A $2500 SIGN-ON BONUS PAYABLE IN TWO INSTALLMENTS - 50% AFTER 90 DAYS - 50% AFTER ONE YEAR

Major Responsibilities:

  • Provision of Patient Care
    • Plans appropriate treatment programs under the supervision of a PT ( depending on area of assignment ) adults of all ages based on physician referral, evaluation results, patient/family goals and interests, sound clinical judgment and other pertinent available information.
    • Administers daily treatment ensuring that the treatment plan is age-appropriate and adjusted as necessary with the assistance of the PT in order to meet the changing needs of the patient.
    • Successfully completes all competency assessments pertaining to provision of patient care.*
    • Selects appropriate assistive devices for patients and provides appropriate instruction in the maintenance and use of such equipment.
    • Communicates with team members, other professionals, family members and caregivers for the purpose of sharing information that will assist in achieving optimum care for the patient. Establishes effective rapport with patients, family members and other professionals.
  • Patient and Family Education
    • Provides education to the patient and family, or significant others based on patient needs. Identifies and addresses learning readiness, preferences and learning barriers.
  • Discharge Planning
    • Consults with physical therapist to determine when the patient will be ready for discharge. Recognizes the need for termination of treatment and effectively plans discharge which may include establishing home treatment plans, arranging for equipment or devices, and/or arranging for continued services.
  • Documentation
    • Completes all written documentation according to departmental policy, and obtains the co-signature of the supervising physical therapist when indicated.
  • Evaluation and Goal Setting
    • Establishes treatment goals in conjunction with the physical therapist.
    • Consults with a PT in interpreting the results of reassessments as necessary according to the complexity of the case and demonstrated clinical competence of physical therapy assistant.
  • Daily Operations
    • Responsible for maintaining a productive schedule, with an average of 6.5 out of 8 hours per day scheduled for direct clinical care. Informs supervisor of openings and/or cancellations in their daily schedule.
    • Performs other duties such as billing for all services rendered on the day delivered, maintaining a clean work environment, and serving on departmental committees and in program development efforts as appropriate.*
    • Participates in the continuous improvement of the quality of services delivered through the department by participating in measuring critical functions, and participating in teams to improve work processes.
    • Follows appropriate safety and infection control measures pertaining to patient care, equipment and the general working environment. Promptly reports any problems to their immediate supervisor and follows appropriate Advocate Condell policies and procedures.
    • Actively participates in departmental meetings, in-services and continuing education programs
  • Delegation/Supervision of Assistive Personnel and Students
    • Supervises physical therapy assistant interns at the request of the supervisor after completing one year of clinical experience


Licensure:

  • Physical Therapist Assistant


Education/Experience Required:

  • Associates degree from an accredited program for physical therapy assistants.


Knowledge, Skills & Abilities Required:

  • Good interpersonal skills and communication
  • Licensed or eligible in the state of Illinois CPR Certification AHA BLS within 30 days of hire


Physicial Requirements and Working Conditions:

  • Required to work approximately 1 weekend per month or evenings depending on area of assignment. Lifts 50 to 100 pounds occasionally and/or 25 to 50 pounds frequently and/or 10 to 20 pounds constantly.
  • If position has direct patient care or direct patient contact the following lifting requirement supersedes any previous lifting requirement effective 06/01/2015. Ability to lift up to 35 pounds without assistance. For patient lifts of over 35 pounds, or when patient is unable to assist with the lift, patient handling equipment is expected to be used, with at least one other associate, when available. Unique patient lifting/movement situations will be assessed on a case-by-case basis.
