The pharmacy residency is a twelve month postgraduate training program designed to prepare pharmacists for practice. The resident will serve primarily in a learning capacity within the institution and participate in patient care across a variety of practice settings. In addition, the resident will participate in central drug distribution via staffing shifts in the central pharmacy.

JOB DUTIES - It is expected that all duties and responsibilities of this position will be performed in a manner consistent with the mission and core values.

  • Competency Area: Patient Care
    • In collaboration with the health care team, provide safe and effective patient care to a diverse range of patients, including those with multiple co-morbidities, high-risk medication regimens, and multiple medications following a consistent patient care process.
      • Interact effectively with health care teams to manage patients' medication therapy.
      • Interact effectively with patients, family members, and caregivers.
      • Collect information on which to base safe and effective medication therapy.
      • Analyze and assess information on which to base safe and effective medication therapy.
      • Design or redesign safe and effective patient-centered therapeutic regimens and monitoring plans (care plans).
      • Ensure implementation of therapeutic regimens and monitoring plans (care plans) by taking appropriate follow-up actions.
      • Document direct patient care activities appropriately in the medical record or where appropriate.
      • Demonstrate responsibility to patients.
    • Ensure continuity of care during patient transitions between care settings.
      • Manage transitions of care effectively.
    • Prepare, dispense, and manage medications to support safe and effective drug therapy for patients.
      • Prepare and dispense medications following best practices and the organization's policies and procedures.
      • Manage aspects of the medication-use process related to formulary management.
      • Manage aspects of the medication-use process related to oversight of dispensing.

    • Demonstrate ability to manage formulary and medication-use processes, as applicable to the organization.
      • Prepare a drug class review, monograph, treatment guideline, or protocol.
      • Participate in a medication-use evaluation.

MINIMUM EDUCATION, EXPERIENCE, TRAINING, AND LICENSURES REQUIRED

Pharmacy licensure in the Commonwealth of Kentucky is required for participation in the residency program. Specific requirements for licensure may be obtained from the Kentucky Board of Pharmacy. Prior to participating completion of human resources orientation is required. All incoming residents are required to obtain a Kentucky intern license (if they do not already possess via completion of Advanced Pharmacy Practice Experiences in Kentucky). The resident will seek Kentucky Pharmacist licensure at the earliest available testing opportunity. Failure to obtain Kentucky licensure by July 31 will result in a restructuring of the PGY1 Pharmacy Residency experience as determined by the RPD and RAC. Should this situation arise, all written orders must be counter signed by the preceptor and all order entry activities must be completed in an unverified state until licensure is obtained. This will ensure compliance with both state and federal regulations. The resident is subject to termination and release of from the PGY1 Pharmacy Residency Program for failure to obtain Kentucky licensure within 90 days from the residency start date. Extenuating circumstances will be evaluated.
