Job Description

The Home Health Speech Pathologist evaluates, plans, and administers comprehensive speech therapy modalities/regimens as prescribed by a physician to patients with speech, voice, language, cognitive-communicative and oral-pharyngeal swallowing disorders in their place of residence for the purpose of improving and/or restoring speech and/or swallowing functions.

Principal Responsibilities

  • Provides initial and ongoing assessment to determine level of function, and any other assessments as required by federal guidelines for patients who have voice, memory, communication, and/or swallowing deficits.
  • Establishes an in-home treatment plan in coordination with the physician and other care team members to develop needed skills of their patients, to restore function.
  • Responsible for planning, evaluation, and treatment implementation of an assigned caseload.
  • Provides quality, personalized, evidence-based care, and proven interventions of Speech Language Pathology.
  • Educates, collaborates with, and conferences with patients, families, and other health care providers; making recommendations for referrals to specialists as needed.
  • Supervise Speech Therapy students and other home health staff as appropriate.
  • Completes required documentation to complete the patients visits and episodes as required by federal, state and payor guidelines.
  • Instructs patients and family/significant others in home programs.
  • Demonstrate excellent knowledge of principals and practices of SLP.

Education/Experience

  • Successful completion of a program approved by (American Speech Hearing Association), ASHA.
  • Master's degree or greater in Speech Language Pathology.
  • Completion of Certificate of Clinical Competence (CCC).
  • One year experience in hospital or Home Care Setting, preferred.

Licenses/Certificates

  • Reliable means of transportation, valid IL driver's license and vehicle liability insurance required
  • Current ASHA license.
  • AHA CPR certification required

  • Strong organizational and time management skills
  • Analytical ability to administer and evaluate standardized speech/language tests, conduct swallowing evaluations, evaluate patients' capabilities, develop treatment plans and monitor patients' progress.

Working Environment

  • This job operates in a variety of settings, including but not limited to professional office environment, patient's homes, and car .
  • Routinely uses standard office equipment such as computers, phones, photocopiers, filing cabinets and fax machines.
  • Must be able to function in a wide variety of environments which may involve exposure to allergens and other various conditions
  • Possible exposure to blood, bodily fluids and infectious diseases
